I guess that can be arranged, However, it depends on your tour operator. There are 3 parts of the tour. 1. Reach the port in sabang which is approx 80 KM from PP. takes 1.5 hours 2. Then get a ride via sea (this was scary for us as 6 people share a boat), then you reach the island, takes 30 minutes. 3. There you go through with the tour with headsets mentioning details on the boat. So if a you take a package to go through with all these, your operator should be able to arrange a boat for 2 of you.

If you are going to do-it-your own tour. local and international have the same rate: Php 735 which includes the entrances, envi. fees, and audio device. additional rate will be the boat ride 2 way: Php 180+.
